Patient online Access

Dalton Surgery already offers patients to book and cancel appointments and order repeat prescriptions online (Patient Access) we are also working towards providing patients with the facility to access summary information from their medical records online.  Summary information means data relating to medications, allergies and adverse reactions

Accountable GP for all patients

As part of the GP Contract changes 2015/16 it was agreed between BMA’s General Practitioners Committee and NHS Employers that all practices need to ensure that each patient on their list is assigned a named, accountable GP.

Dalton Surgery would like to  inform patients that they have a named GP who is responsible for patients’ overall care at the practice, that they should contact the practice if they wish to know who this is, and that if they have a preference as to which GP that is, the practice will make reasonable efforts to accommodate this request.”

ZERO TOLERANCE

The NHS operate a zero tolerance policy with regard to violence and abuse and the practice has the right to remove violent patients from the list with immediate effect in order to safeguard practice staff, patients and other persons. Violence in this context includes actual or threatened physical violence or verbal abuse which leads to fear for a person’s safety. In this situation we will notify the patient in writing of their removal from the list and record in the patient’s medical records the fact of the removal and the circumstances leading to it.

NHS Calderdale Clinical Commissioning Group (CCG) and NHS Greater
Huddersfield CCG have launched a public consultation on proposed changes
to hospital and community health services in Calderdale and Greater
Huddersfield.
The consultation started on Tuesday, March 15th and will run until midnight on
Tuesday June 21st.
Under the proposed changes, all emergency, acute and high risk planned
care would be brought together at Calderdale Royal Hospital (CRH) and the
Acre Mills, Huddersfield, site would be developed as a hospital dedicated for
planned care.
The proposed new model of care is supported by further strengthening
community services.
Details of how to complete a survey online or information about the proposed
changes, consultation materials, and details of drop-in information sessions
and public meetings are available on the Right Care, Right Time, Right Place
